You know that feeling when something big is about to go down? Episode 49 of "It's Okay to Not Be Okay" has that vibe all over. Patrick (Joshua Garcia) is starting to resist his 'Manong' Matmat (Carlo Aquino), and it’s giving us the sense that any day now, Matmat will have to finally face the trauma he’s been running away from.
But first, let’s talk about those 50 orders of inasal that Mia (Anne Curtis) was forced to buy from Jason (Francis Magundayao)'s restaurant just to get a chance to talk to Matmat on duty. But even after all that, and even after she goes as far as threatening him with breach of contract for refusing to be her illustrator, nothing can seem to convince Matmat to be her 'bestie' again. So, Mia heads to OK Hospital and gives the food away.
Patrick laughs at the whole situation, but he also reminds Mia that threats won’t win Matmat over. If she really wants to reach him, she has to earn his trust. And since the brothers can’t go back to the mansion yet to watch over her, Patrick leaves Mia with instructions that sound a lot like boyfriend duties just to make sure she stays safe while she’s on her own.
But of course, he can’t stand the thought of Mia being all by herself, so he ropes Madeth into being her new sidekick. And surprise, surprise, they actually get along, though not right away since Mia only agrees to it after realizing it’s Patrick's idea.
Mia looks happy with how things are going with Patrick, but reality is about to hit her hard. OK Hospital turns somber as the staff and patients learn that her father Samuel (Michael De Mesa)'s cancer has spread beyond the point of treatment. The nurses, doctors and patients alike show sympathy. Trust Eden (Agot Isidro) to catch the details as she notices how Olivia (Maricel Laxa) seems especially emotional about Samuel's condition.
Later, Zelda (Ana Abad-Santos) storms back in the facility, hysterical and terrified and repeating that she doesn’t want to die. The nurses rush to calm her down, but until then, she hasn’t said a word about what Dipong did.
Meanwhile, Doc Anok (Bobot Mortiz) and Patrick take a closer look at Matmat’s mural and notice how he still avoids painting butterflies. Doc Anok and Patrick are just too cute to watch together. It's hard not to love this pairing. Patrick says coming back to Maravida for therapy was the right choice since Doc Anok was the only one who really tried to use Matmat’s artistic gift in the process. Touched by Patrick's trust, the doctor quips, "Pinababa ko nga blood pressure ko, pinatataba mo naman ang puso ko." How adorable is that?
Doc Anok later comes up with a tactic to make Matmat confront his fear of butterflies, telling him he won’t be paid for his mural unless a butterfly appears on the painting. Offended, Matmat storms off early and calls Doc Anok a scammer.
At home, Patrick scolds Matmat for walking away from a job he already committed to. This sets Matmat off into a full-blown tantrum. He tries to lock himself in the closet, but Patrick holds him back. Desperate to break free, Matmat bites Patrick’s arm, forcing Patrick to fight back until the struggle turns almost childlike and physical.
Patrick freezes for a moment after realizing this is the first time he’s fought back against one of Matmat’s outbursts instead of enduring his tantrum like he always did. And Matmat is just as stunned to hear his brother raise his voice.
