Fresh from their heart-stopping kiss, Patrick (Joshua Garcia) and Mia (Anne Curtis) step into "It's Okay to Not Be Okay" Episode 48 with feelings too strong to hide, as he pleads with her to stay by his side while he continues to look after his 'manong' Matmat (Carlo Aquino).
Patrick asks for a conversation with Mia, where he reveals that Matmat’s trauma stems from witnessing their mother Tess (Meryll Soriano)'s murder—the only thing he remembers from that night being “butterflies.” He recalls how they left Maravida after overhearing that social workers were planning to separate them, but what he didn’t expect was how deeply Matmat’s grief would manifest.
Every death anniversary of their mother, with the start of the rainy season, Matmat’s memories get triggered that he becomes terrified of butterflies, convinced they will come to take him away. This fear had forced them to uproot their lives and keep moving from one place to another each year. Hearing this, Mia finally understands why Matmat dreams of having a camping van. But she wonders how a butterfly could possibly kill the brothers' mom.
Even so, Patrick asks Mia to remain in his life. He knows Matmat will always be his priority, but he also believes Mia is destined to be part of his story. He wants her to stand beside him, the way he stands beside Matmat.
At OK Hospital, patient Cynthia (Alora Sasam) teases Patrick about the glow on his face "na para bang may happy filter." Joshua and Alora's brief exchange feels so natural, especially when she tries to make him smile and Joshua looks away, all shy-boy vibes.
Cynthia's 'chika' then reaches Doc Anok (Bobot Mortiz), who also teases Patrick about his glowing aura. Patrick laughs it off, insisting it’s nothing more than waking up on the right side of the bed. "Sana all," because unlike him, Olivia (Maricel Laxa) and Eden (Agot Isidro) chose violence that morning.
Drama flares up when Eden insists on clearing the bed of escaped patient Zelda (Ana Abad-Santos), arguing it is more practical to admit a new patient from the waiting list, which Olivia strongly opposes since Zelda is still under her care. Words fly and the exchange grows heated as Eden provokes Olivia about finally having more free time to dote on Samuel (Michael De Mesa).
This triggers Olivia to slap Eden, who fires back just as quickly. Their spat escalates into a physical clash until Doc Anok jumps in like a referee. Left alone after the catfight, Eden begins to wonder why Olivia is always so easily triggered whenever Samuel is mentioned. Now she's convinced that she’s close to uncovering whatever secret her rival has been keeping.
Over at Jason’s Inasal, Matmat gets a shock when Mia walks in as a customer. But instead of ordering from the menu, this customer's request is for Matmat and Patrick to return to her house. She then tries to present Matmat, still her illustrator, her new story idea about three souls on a journey to find what really matters.
But Matmat’s stubbornness pushes Mia to desperate measures. She threatens legal action and even demands ₱100,000 in damages if he decides not honor their contract. Still, Matmat cannot be convinced to “bestie” her back. Meanwhile, Jason (Francis Magundayao) treats it as business as usual, protecting Matmat, who is visibly uncomfortable with Mia's persistence, while also cashing in when she randomly orders 50 servings of inasal just to secure entry into the restaurant.
From left to right, Episode 48 serves up nonstop drama—fights, secrets, and plenty of kilig. But the teaser warns us that things won’t stay sweet for long, with Patrick and Matmat headed for another falling out. Will this brotherhood survive yet another test, or are we watching it slowly fall apart?
