One song is all it takes to set Samuel (Michael De Mesa) off in Episode 45 of “It’s Okay to Not Be Okay.” At OK Hospital, he’s taking a quiet moment in the garden when a woman’s humming cuts through the silence. In his mind, the humming overlaps with an eerie lullaby he used to hear from his wife Ingrid (Empress Schuck). The lullaby from his past now blurs with the tune being sung in the present by Zelda (Ana Abad-Santos).
“Papatayin mo ba ako ulit?” chillingly asks Zelda as Samuel closes in, before he snaps and strangles her. The scene is interspersed with flashback of Ingrid and young Mia (Ysabella Suarez) suffering the same fate from Samuel (Geoff Eigenmann). Patrick (Joshua Garcia) catches the horrifying act and pulls Samuel away, but is left rattled when Samuel issues a warning that Mia (Anne Curtis) and Ingrid will kill him if he doesn’t kill them first.
Patrick and Doc Anok (Bobot Mortiz) pore over the CCTV footage, puzzled why Samuel attacked Zelda. Was it just mistaken identity, with Samuel confusing Zelda for his wife, or is there something more? Doc Anok dares to voice a darker suspicion—that Zelda might hold dangerous knowledge about Samuel, or that maybe she's Ingrid herself.
He brushes it off as mere speculation, citing how Ingrid’s body was never found despite her supposed death. But the theory unsettles Patrick, who remembers Dipong’s chilling warning that he and Matmat (Carlo Aquino) must leave the mansion because Ingrid will return.
And as viewers already debate whether Olivia (Maricel Laxa) or Eden (Agot Isidro) could be Ingrid, here's Zelda adding another layer of mystery.
Across town, Nanay Li (Rio Locsin) comes home to find Mia waiting outside like a child in need of food. She recalls Nanay Li once told her she could drop by anytime she wanted to eat. Mia is stunned when Nanay Li greets her a happy birthday, to which Nanay Li explains that she just knew because anyone who asks for spaghetti must be celebrating their birthday.
Nanay Li excuses herself, saying she’ll whip up a quick fruit salad, but soon returns with Vincent (Enchong Dee), Madeth (Xyriel Manabat), and Jason (Francis Magundayao), all singing “Happy Birthday” while bringing out food, balloons, and a chocolate cake (which Madeth jokes is as dark as Mia’s conscience).
Vincent gifts his favorite writer with a piece of jewelry, which she appreciates since he knows her taste so well. He also knows the longing on her face from missing Patrick and Matmat. He tells her it’s time to wake up from her dream and end her obsession over the brothers so she'd leave Maravida and her past will no longer weigh her down. Vincent points out that the Montanesa trip with Patrick is memorable only because it was temporary.
Mia cannot believe that Vincent is speaking with such wisdom, almost as if he now wants to be a writer like her. Before they could argue, she declares that she is finally going to begin writing her book, that’s why Vincent cannot stop Mia from going to Patrick and Matmat’s house inside the compound.
Only Matmat is home, and he wants nothing to do with her. She tries everything she can to soften him—returning Satsat, calling him "bestie" and even tricking him by pretending to break down the door with a chainsaw—but nothing works. Matmat locks the door and scoffs at her for calling him “bestie.” Because what kind of best friends would lie to each other? Matmat still can’t forgive Mia for bringing Patrick to Montanesa without telling him.
Left outside the door, Mia reminds Matmat of her story "The Boy Who Cried Wolf," about a boy who repeatedly cries out that a wolf is attacking his flock. Each time the neighbors come running, they find no wolf at all. Still, the boy calls out and lies repeatedly just because he's lonely.
Back at OK Hospital, Patrick pushes through his night shift even while feeling sick, relying only on the medicine Gemma (Kaori Oinuma) gave him, until an emergency throws them into panic. They discover Pogi (Aljon Mendoza) lying unconscious on the ground just outside, with a syringe and vial beside him that strongly suggest he has been injected.
