At St. Benedict Hospital, a new patient named Alex (Christian Vasquez) was admitted to the psychiatric ward after a suicide attempt, having overdosed on alprazolam, a medication prescribed for anxiety disorders. What made the case even more alarming was the information relayed to Patrick (Joshua Garcia) that Alex had also tried to make his young daughter Mona (Briseis Quijano) take the same pills, an act that pointed to a shared suicide plan. And while Alex was placed under clinical observation, Mona was being monitored separately for signs of PTSD.

Later, Alex managed to slip out of the ward and wandered around the hospital in search of Mona, who, as it turned out, was seated at the auditorium, listening to Mia Hernandez (Anne Curtis) reading her children’s story to a full audience.

In the middle of Mia’s narration about a boy being haunted by a spirit of nightmares named Batibat, the lights suddenly flickered on, cutting the event short as security and the nurses, led by Patrick, had begun a search for the missing patient.

Ever her usual self, Mia demanded that the security ask the audience to return to their seats and resume the event, but no one listened. Mia then found Alex and Mona backstage, and she overheard Alex telling his daughter that it's time to go through with their plan to die together, but Mona, scared, said she isn’t ready, only for Alex to respond that being left alone would be even worse and that she wouldn’t survive without him.

Mia stepped in and called Alex out for being selfish, saying it isn’t Mona who's afraid to be alone—it's him. Alex, provoked and spiraling, snapped and charged at her, but Mia managed to punch him in the face and knock him to the ground.

Disoriented but desperate, Alex reached for a knife that fell from Mia’s bag, and in a sudden turn, Mia lost her balance and fell, giving Alex just enough strength to pin her down and start choking her. In that terrifying moment, Mia froze as memories from her past came rushing in. A flashback showed a younger version of Mia being pinned to the floor by her own father, who was trying to kill her.
