One night at OK Hospital, Samuel (Michael De Mesa) hears footsteps and follows them outside his room into the hallway, where exhibits an altered state of behavior after spotting a woman in black singing.

The next morning, Patrick (Joshua Garcia) finds Pogi (Aljon Mendoza) staring blankly, drained from fatigue after he and the other night-shift nurses had to deal with Samuel, who was later discovered unconscious outside his room.

Patrick takes care of Samuel that morning when the patient suddenly asks if he also heard the singing woman. Samuel then mutters, “Patay na siya. Bakit nandito siya?” without mentioning any name. Patrick tries to clarify if he’s referring to his wife, but Samuel falls silent. This leaves Patrick unsettled, realizing Samuel’s words echo Dipong's warning that Ingrid (Empress Schuck) would return, even more troubled by the fact that her body was never found after she was declared dead.

Meanwhile, Doc Anok (Bobot Mortiz) reviews the CCTV footage to investigate what happened to Samuel. In the video, the hum of a woman can be heard—the same melody Samuel had been singing, as previously recounted by Tommy (Bodjie Pascua). Zooming in, he spots a woman dressed in black.

The scene cuts to Doc Anok questioning Zelda (Ana Abad-Santos) if she saw Samuel the previous night. She claims she only stepped out to get some air and saw nothing unusual, though visibly uncomfortable when asked if she had been humming during that time. Standing nearby, Patrick listens closely as he keeps watch over Zelda during his duty.
